Poland Tourist/Family visit Visa Checklist

     
Passport – Issued within the last 10 years, valid for at least 3 months post-departure from Schengen, with at least 2 blank pages.
Application Form – Fully completed, signed twice, with the signature matching that on the passport. Applicant must be at least 18 years old to sign.
   
Photos – Two passport-type colour photos, 35mm x 45mm, white background, no older than 6 months.

Cover Letter – Stating purpose, duration of travel, and list of attached documents.

Confirmation of organized trip booking or travel plan.

Visiting Family/Friends– Invitation letter from host, including their address, contact details, and stay duration.

Proof of host’s legal residence valid for the entire stay (residence permit, visa)

If host provides accommodation: proof of right to accommodate guests (e.g., rental agreement)

Family Visits: Proof of family relationship (e.g., legalized birth certificate).
Friends Visits: Documentation to confirm friendship (photos, etc.).

Flight Itinerary – Reservation only; includes proof of intra-Schengen travel (e.g., train itinerary).

Travel Medical Insurance – Original certificate from an approved provider, valid for the entire Schengen area and travel period. Minimum coverage: €30,000 for emergency medical, hospitalization, and repatriation.

Accommodation – Original confirmation of stay in Poland or each visited Schengen country for the travel period.

Proof of Financial Means:

Bank statement (last 3 months), stamped and signed by bank

Income tax returns for last 2 years

If employed: recent pay slips (3 months), employment contract, employer holiday approval

If self-employed: Business registration certificate, GST registration (for Indian applicants)

If retired: Pension statements (last 3 months)

For minors: Proof of financial means from parents/guardians.

Minors: Travel permission from absent parent(s)/guardian(s), legalized.

Copy of parent/guardian identification documents.

Additional Documents – Any further documents the applicant believes support the purpose of their trip.

Proof of Residence – ID showing residency within Embassy/Consulate jurisdiction.

Passport Data Page Copies – Copy of passport data pages (first and last) and Schengen visa pages if applicable.

 

Processing Time: 15– 20 working day (depends upon the embassy).

 

Additional Notes:

  1. Embassy/Consulate Rights: The Embassy/Consulate may request further information or documentation or conduct an applicant interview if necessary.
  2. Processing Time: Applications are processed within 15 calendar days, with the possibility of up to 45 days in some cases. Applications submitted within 45 days of travel may carry processing risks.
  3. Jurisdiction: The Consulate General of Poland in Mumbai accepts applications from the following VAC locations:
    • Mumbai, Ahmedabad, Bangalore, Chennai, Hyderabad.
    • Jurisdiction includes Maharashtra, Gujarat, Tamil Nadu, Karnataka, Telangana, Kerala, Andhra Pradesh, and others listed in the original notice.
